clutch let go and chain fell off at same time

      ON my bikes i use the sizxe that just fits into the holes snuggly.. to me thst is what i consider the proper size...thats just my thing... i dont allow any play that could let something loosen a bit... the main purpose of the crown nut/cotter pin system is to keep the nut from loosening in the first place and from falling completely off it should for some reason come loose.

        mine is not a crown nut, just standard. someone must have changed it.

          just another FYI thought from my experience..Also check the engine mounts on each side of the bike.. i had a problem once with the chain mysteriously getting loose then tight... with my bikes being hard mounted motors, i found that BOTH of the engine mounts were loose.. this was allowing the engine to shimmy around and was causing all kinds of weird symptoms.. may be nothing on yours but doesnt hurt to check itm out..

            is there a hole drilled in the axle bolt???

                then yes it requires a crown nut and cotter pin..you should have a big flat washer on there too. take the axle bolt to the hardware and get a crown nut that fits...

                    to share our experiences with these old great machines is why we are here.. the thanks is to you for letting our experiences benefit you and others...Like bikecliff says.. periodic bolt inspection is a must...These bikes have there own little nuances and theyn vibrate from all kinds of things... hard mounted engines, chains and such rolloing.. etc etc... gotta keep them tight nad locked down. post back and let us know what you find..OK???
