    sunday morning quarterbacks needed

I just pulled the clutch cover(83 Gs1100e)), pressure plate and all the Disks. The center nut was loose as heck 8O FIRST EVIDENCE BEING THAT THE PRESSURE PLATE ROCKED A BIT BEFORE I LOOSENED ANYTHING :? . The peened over washer is all that kept it from coming off. I have had a billet nut and washer instslled for at least 3 years. Usually when i dismantle anything i have put in myself everything is tight because i use a torque wrench.
Naturally i could have made a mistake on the torque. Are there any other reasons why this could have happened? The inner basket is new and the outer was done to Hd specks and ballanced a when i did the clutch work way back when. Possibilities please.

The tab on the lock washer is probably worn or broken off allowing it and the nut to turn USE some locktite when reassembling
